Consent for ABHA creation

Please read and agree to the ABDM consent before proceeding.

1I, hereby declare that I am voluntarily sharing my Aadhaar number / Driving Licence and demographic information issued by UIDAI / the issuing authority, with the National Health Authority (NHA) for the sole purpose of creating an Ayushman Bharat Health Account (ABHA) number and ABHA address.
2I understand that my ABHA number / ABHA address can be used and shared for purposes as may be notified by ABDM from time to time, including provision of healthcare services.
3I authorise NHA to use my Aadhaar number / Driving Licence for performing Aadhaar / document based authentication for ABHA creation. I understand that NHA shall ensure security and confidentiality of my personal identifiable information for the aforementioned purpose.
4I have been duly informed about the option of using other IDs apart from Aadhaar for creation of the ABHA number; however, I consciously choose to use my Aadhaar number for the purpose of availing benefits across the National Digital Health Ecosystem (NDHE).
5I consent to usage of my ABHA address and ABHA number for linking of my legacy and future health records, and for sharing of these records with the requesting health information user, only with a valid consent provided by me, for the purpose of availing health services.
6I am aware that I have the option to withdraw my consent at any point of time, and that my health records will not be shared further without my consent.
